The Soberlink team can help you with all client record related requests.

At Soberlink, we are equipped to provide various types of records without necessitating a subpoena*, given that the requester is listed on the monitoring agreement as a Monitored Client, Concerned Party, or Contact. These records include:

  • Client Detail Reports
  • Monitoring Agreements
  • Change Order Requests

*May be subject to additional fees

Monitored Client, Concerned Party, or Contact on the Monitoring Agreement: Choose and complete either Option 1 or Option 2 below.

Legal Professionals Not Listed as a Contact on the Monitoring Agreement: If you are a legal professional and your name is not included as a Contact on the Monitoring Agreement, instruct your client to follow the instructions for Monitored Clients, Concerned Parties, or Contacts as outlined. You cannot complete these options on behalf of your client without a subpoena. If you would like to serve a subpoena, select Option 3.

Client Testing Records Request

‍If you only need a copy of client testing records, you can obtain these by going to any email from noreply@soberlink.net that contains a Daily, Weekly, or Monthly report. Click on the “Download Full” link in the top right of the document.

Option 1: Non-Authenticated Client Record Request

For records that do not require a notarized business records affidavit, you can easily download reports from the MySoberlink Portal.

Option 2: Authenticated Client Record Request

If you are requesting records that require a notarized business records affidavit, fill out the form below. 

Option 2 Fees: There is a fee of $50 for authenticated records, which is the responsibility of the Requesting Party.

Please note: You must be a Monitored Client, Concerned Party, or Contact on the Monitoring Agreement to receive these reports.
